Note: During the webinar, there was a question about whether a specific form is required in Massachusetts for a medical or religious exemption to immunization. Please note the following clarification: (1) a medical exemption is allowed if a physician submits documentation attesting that an immunization is medically contraindicated; and (2) a religious exemption is allowed if a parent or guardian submits a written statement that immunizations conflict with their sincere religious beliefs. Some schools may require that the exemption be submitted on a specific form, but this is not a requirement at the state level.
